Gen'l, Commanding Dept of N York" as an endorsement of the reverse of the printed Certificate for discharge, February 4, 1862. Private David Inaustin of the 91st NYSV is discharged for "Large oblique hernia of the right side. He was left at Fort Columbus and should never have been enlisted" attest Surgeon W. I. Sloan. Additionally, the document is endored on the reverse by "G(ustavus) Loomis, Col. 5 Inf, Cmdg"
