War Department document related to the death of Pvt. Nelson Davis, Co. F, 12th New Jersey Vols. who died of dysentery at the infamous Confederate POW camp at Andersonville, Georgia. The pre-printed, 3pp. 4to., War Department, Adjutant General's Office, Washington, April 28, 1865 form calls for the necessary paperwork to be filed with the department in order that the U. S. military could finalize its accounts for Private Davis who was captured at Spotsylvania Court House, Va. in May 1864. He died at the cruel prison camp just two months late and is buried at Andersonville National Cemetery in plot number 3444. Interestingly, a sergeant in Davis' company, did not remember his unfortunate comrade when, as commander of Co. F, he was asked to supply the necessary paperwork. The inside of the document contains a great example of instructions of a, "Model For Set of Papers For A Deceased Soldier."…PLUS; docket panel "Adjutant General's Office, Washington, July 31, 1865," asking that "this paper to be returned with other papers" by Company F's commander.
