War-date Confederate partly-printed Document Signed, 1p. quarto, November 19, 1862, Atlanta, Georgia, stating that Henry Polk, of Company C, 7th Regiment of Florida Volunteers, was paid $63.90 for his service from April to September 1862. Signed by Captain Massengale and by Polk with his "X". VG. Henry Polk was mustered into the 7th Florida Infantry on April 26, 1862. He was severally wounded at Taylor Gap, Georgia on February 25, 1864, and wound eventually surrender with Johnston at Durham Station, North Carolina. He was paroled at Greensboro, May 1, 1865. He participated in the Battles of Chickamauga, Chattanooga, Atlanta Campaign & Siege, Franklin, Nashville, the Carolina's Campaign and Bentonville.
