Complete Manuscript Document, 8" x 12", 1pp., with intact seal and stamp, January 15, 1864. The affidavit is from "State of Indiana", stating that "Dr. C. Richmond, who upon his oath says that he has carefully examined Major Willis Blanch of the 57th Reg Ind. Vol. and finds him unfit for duty because of a Gunshot Wound of the left hand and he will be fit for duty for twenty days. ..."
