March 26, 1932, (Prohibition) Prescription Form for Medicinal Liquor, Baltimore, Very Fine. Pink, 4.5" x 5.5", "Duplicate" form is a prescription for whiskey for a patient, Lillian Bramblett, of Baltimore, Md. Along the left side is printed "Bureau of Industrial Alcohol", and along the right is, "U.S. Treasury Department" and at bottom, "Issued Under Authority of the National Prohibition Act." Two holes punched at left side, otherwise like new.
