Preprinted form, no date, filled out in manuscript, 8" x 12", on Confederate States of America Treasury Department letterhead, April 4, 1864, certifying "the claim of James Johnson father of Alexander Johnson, a private in Captain Boyd's Co. (G), 19th Regiment Va. Infantry ... the sum of One Hundred, Eighteen dollars and Twenty-six cents for services of said deceased ..." The form continues with the approval of the Comptroller and Chief of Clerk with their ink signatures. Albert Alexander Johnson, on 5/1/1861 mustered into "G" Co. VA 19th Infantry . He was Killed on 6/27/1862 at Gaines' Mill, VA.
